NOTE: Please submit one form per course. Do NOT submit multiple Permit Request Forms for the same course. Doing so may result in not receiving a permit. This form is for FCBE Special Programs (MS Accounting, MSBA-Finance, MA Economics) and PhD Students ONLY. Email requests are no longer accepted.

All requests for permits outside of FCBE should be submitted through the respective college (e.g., for COMP 7012, you will need to contact the Department of Computer Science).

Requests should only be submitted for FCBE courses that are listed as “Permit Required.” Please confirm this requirement before submitting this request form.

Doctoral Students:
Dissertation (BA 9000) and independent study course permits will only be issued to those who have been added to the approval list by their PhD coordinator. The number of hours requested must be included on the approval list.
