Students interested in pursuing a College of Professional & Liberal Studies undergraduate degree must complete our Declaration of Major form. Once you are coded to your chosen degree, you will be assigned a permanent College of Professional & Liberal Studies advisor. Please allow 3-5 business days for the form to be processed

Please do not complete this form if you are not a current student or if you have not yet been admitted/readmitted to the University. We are unable to process forms if not admitted.

Note: Grade disputes and course withdrawals during semesters under your previous major cannot be processed through the department of your new major.
