Instructions for Library Media Specialist Practicum Application
 
The following materials are required to be eligible for the Library Media Specialist Practicum Semester. 

1. PRAXIS II Scores/Registration – It is required that you pass ALL required Praxis II exams in your licensure area. You must submit a copy of the scores to the Office of Clinical Practice no less than 30 days prior to the 1st day of the practicum semester. If you are obtaining an add-on endorsement as a licensed teacher, then you would only need to take the Library Media Specialist Praxis II exam(s). Please check with www.ets.org.

You must include either a copy of your test scores or a copy of your registration for the tests with this application. 

2. TBI Criminal Background Check– All candidates must submit a VECHS Waiver and obtain a background check. Please click here for further directions. If you are already working as a classroom teacher or school librarian, you may forgo obtaining a background check by submitting a copy of your school ID badge (if applicable) and a copy of your most recent paystub to this application. 

3. Preferences – If you currently hold a teaching license, then you will upload a letter from your principal indicating that you are approved to complete the required 200 hours at your school. If you do not currently hold a teaching license, then you will complete the Clinical Teaching application.
