• ONLY request MIS courses using this Permit Request form.

• Submit one form per course.

• Request does not guarantee permit. First come first serve basis. Once courses are full, you will be placed on a Department waitlist that is managed by Anna Myers.

• For Fall & Spring: Please do not request permits for more than 4 courses. It is suggested to take no more than 3 courses, especially in your first semester. If you are wanting more than 4 courses, you will need to email Anna Myers.

• For Summer: Please do not request permits for more than 2 courses. If you are wanting more than 2 courses, you will need to email Anna Myers.

• For Fall & Spring: If you are a F1 VISA student: You MUST take at least 3 courses. 2 of those courses must be ON CAMPUS. The 3rd course can be online OR on campus. If you take 4 courses, 2 of those courses must be ON CAMPUS. The 3rd and 4th courses can be online OR on campus.

• For summer: If you are a F1 VISA student: there are no course hour restrictions or online versus ON CAMPUS restrictions unless summer will be your first semester. If it is your first semester, you must take at least 2 courses. One course must be on-campus and the 2nd course can be online OR on-campus.
