The following brief demographic form and questionnaire is designed to explore your perceptions of your ability to counsel children ages 5-12. After completing the questionnaire, a debriefing form will be delivered that gives more information on the intent and purpose of the research. Before you decide whether to accept this invitation to take part in the study, please ask any questions that might come to mind now.
